ENROLLMENT

Students enrolling in Madison Middle School will need the following documents:

*Enrollment Sheet

*Certified Birth Certificate

*Updated Immunization Certificate

*Physical Examination (within the past year & required for 6 th gr. entry)

*Proof of Residency

*Custody Papers, if the student does not live with both biological parents

COUNSELING SERVICES

Individual: Referrals may be made by students, teachers or parents. Please call for an appointment or leave a written request at the school office.

Small Group: Based on student need. If a student is invited to participate in a group, parent permission will be obtained

Classroom Guidance: Academic, Social/Emotional and Careers

We also have a collaborative relationship with Blue Grass Regional Mental Health Comprehensive Care. Therapists from Comp Care meet with students two mornings a week, Impact Services monitor students regularly and Children's Wrap Around coordinate services. Services from Blue Grass Regional Mental Health must meet a specific criterion with parent involvement. Call the Counselor's Office for additional information

Most semesters, we have counselor interns and practicum students from EKU that provide counseling services for our students

 

BUILDING ASSESSMENT COORDINATOR (BAC)

6 th , 7 th and 8 th graders will take the MAP test 3 times per year (September, January and May). All students also take the CCT in April. Eighth Graders will take the EXPLORE Test in September which counts 5% of the CCT score. The EXPLORE Test is the first test in the EPAS which allow students to begin their planning for high school and college courses. All students will begin or continue work on the Individual Learning Plan (ILP). The ILP is mandated by the state. This is a web-based program and you will need a student/parent log-in. Jennifer Hess, our Career/Business Teacher is the manager of our ILP's.

ACADEMIC SUPPORT

If your child is struggling academically or socially, your recommended first course of action is to contact the teacher or request a conference with the team of teachers. (You may reach your child's teachers via email and email address on the school website) Madison Middle School has a well structured Response to Intervention (RTI) program in place. At the end of the previous school year or beginning of a new school year, all students are given Universal Screeners in Reading and Math to determine academic levels. Based on the screeners, Tier 1 interventions are implemented in the classroom. If students are not making sufficient progress, they may be moved to Tier 2 which is a RTI class that replaces one related arts class for a minimum of 9 weeks. If student progress is little a Student Teacher Assistance Team (STAT) may be called to discuss a move to Tier 3 for more intensive interventions and a discussion for a referral for Special Education Services.

REFERRAL FOR A 504 PLAN

If your child has a medical issue that “substantially” limits his/her access to the curriculum or school facilities and may impair their learning, you may make a referral for a 504 Plan through the Guidance Office. A meeting is scheduled to review the referral and concerns and the committee determines any additional information that may be needed such as a diagnosis of the medical condition (required), rating scales, testing, observations, screenings, etc. After all information is acquired, a second meeting is conducted and eligibility is determined. If eligible, a 504 Plan is written and will be implemented by the student's teachers. For more information, contact the counselor who serves as the school level 504 Chairperson.

 

GIFTED AND TALENTED SERVICES

Students that are identified as gifted in academic and General Intelligence areas will be scheduled so that our G/T Teacher can consult with their teachers to enhance their learning with advanced activities and projects. Activities for students gifted in non-cognitive areas will have special projects scheduled throughout the year. Students may also receive services through a component in our RTI Program. SCHEDULING

6 th Grade – One Team of 4 Teachers and One Team of 2 Teachers

7 th Grade - One Team of 4 Teachers and One Team of 2 Teachers

8 th Grade – One Team of 4 Teachers

 

Each student will have 6 class periods. All students will have a math, science, social studies and science class. There are two periods (Related Arts) where students may have the following classes:

Band and Chorus – Both year long courses,

Art, Wellness (PE), Wellness (Health), Music, Industrial Technology, Careers – each have some two-term classes and some one-term classes,

RTI (Response To Intervention); READ 180 (takes both Related Arts periods) – both of these classes are based on student test scores and screenings and may last anywhere from one to four terms.

 

ATTENDANCE/TRUANCY

As a counselor, I feel attendance is very important. If students are not in school, they cannot learn what is needed to have the best education possible. Our county has a strict attendance policy. Three days of unexcused absences is considered truant and 6 days is considered habitual truancy. Our attendance clerk is Donna Martin. It is VERY important that students turn in their notes and doctor excuses when they are absent. The policy states that excuses must be turned in within 3 days of the absence or they will not be accepted. This is necessary so student work can be made up and so that students will be eligible for rewards and recognition. This also teaches students responsibility and that there are consequences when rules are not followed. *Make transition to middle school an adventure

*Shop for school supplies and dress code apparel together-allow them to make choices

*LISTEN – students are going through “new” emotions and experiencing new feelings

*When students become discouraged or conflicts arise, look at both sides
